This is per diem position. Day shift, Monday - Friday. PRN
 

• →Conduct patient assessments and develop personalized educational plans for individuals with diabetes
• →Provide comprehensive diabetes education to patients and their families, covering topics such as blood glucose monitoring, medication management, insulin administration, nutrition, and lifestyle modifications
• →Teach patients how to use diabetes care technologies, including glucose meters, insulin pumps, and continuous glucose monitoring systems
• →Collaborate with the healthcare team to develop and implement treatment plans, making recommendations for adjustments as needed.
• →Offer ongoing support and counseling utilizing motivational interviewing to help patients cope with their condition and improve outcomes
• →Document patient assessments, interventions, and progress notes in accordance with healthcare standards and regulations

• Licensed in the State of Washington as a Registered Nurse
• Minimum of 2 years clinical practice with preferred experience working with PWD
• Certification as a Diabetes Care and Education Specialist preferred
• Current BLS for Healthcare Providers
