What You'll Do In This Role:
Be a trusted helping hand in the moments that matter most. As a Home Health Concierge, you’ll provide personalized, non-clinical support to participants—often one-on-one—in their homes or in care settings such as assisted living or skilled nursing facilities. Guided by the interdisciplinary team, you’ll focus on enhancing comfort, independence, and overall well-being through meaningful, participant-centered support.
