General Function:

Responsible for providing quality medical care, specific to specialty, to NHA patients on both outpatient and in-patient status. 

Position Overview:

The Physician is a member of the (Patient Centered Medical Home) PCMH Care Team. All members of the PCMH Care Team will work cooperatively with and interact with the patients and other team members to coordinate patient care and achieve the stated patient and PCMH Team goals and objectives. PCMH goals and objectives include increased access to patient care, high quality patient care, and patient self-management of care.

Key Responsibilities:

  • Provide medical services as directed by the Medical Director and within the physician’s training.
  • Maintain accurate patient records and comply with all NHA standards, laws, and professional ethics.
  • Complete required medical reports and documentation.
  • Provide in-patient care and assist with coverage arrangements when unavailable.
  • Communicate malpractice insurance status to the Medical Director.
  • Participate in required meetings, quality assurance activities, and peer review.
  • Complete 50 hours of continuing medical education annually as required by Ohio law.
  • Maintain regular on-site attendance.
  • Perform other duties as assigned.

Qualifications:

• Board certified/eligible in field of practice. Experience in working with medically indigent preferred.
