→Education: PM&R residency, fellowship a plus

→Certification: Board Certified/Board Eligible by the American Board of Physical Medicine and Rehabilitation

→Skills: Spine and MSK procedures

→Work with musculoskeletal, sports and spine patients
→Perform spinal interventional procedures
→Serve patients from Endeavor Service Area (Chicago Metropolitan Market)
→Physician will have at least 34 hours per week of direct patient contact
→Physician may be expected to work extended hours
→Flexibility to determine own extended hours (early morning, evenings or weekends)
