Physical Therapist (Home Health)

The home health therapist will be responsible for providing high quality and coordinated care to patients in their homes and communities. The ideal candidate will have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives.

• →Conduct comprehensive physical therapy evaluations and create personalized treatment plans to help patients achieve their goals.
• →Teach patients and their families how to properly perform exercises and activities to promote recovery and improve mobility.
• →Document patient progress, including changes in condition, treatment plan, and goals, and communicate updates to other healthcare professionals.
• →Coordinate care with other medical professionals, including physicians, nurses, social workers, and speech therapists, to ensure the most effective treatment is being delivered.
• →Adhere to established policies and procedures, including infection control and safety protocols.
• →Educate patients and their families on injury prevention and self-care techniques to reduce the chances of relapse or recurring injuries.
• →Maintain accurate and thorough records of patient interactions and treatment plans in accordance with state and national regulations.

• Graduate from an accredited physical therapy program with a valid Physical Therapy license in the state of Florida.
• Minimum of one year of experience in physical therapy, preferably in a home health care setting.
• Demonstrated ability to create effective physical therapy treatment plans that promote patient recovery.
• Effective communication skills, both verbally and in writing, to effectively communicate with patients, families, and other healthcare professionals.
• Ability to work independently and collaboratively as part of a larger healthcare team.
• Ability to read and interpret physician orders and implement treatment according to orders.
• A passion for helping patients achieve their best possible outcomes, through compassionate care and evidence-based treatment.
