Responsibilities

→Assist in creating and implementing education curriculum with Clinical Education department.
→Demonstrates techniques and best practices for trainees.
→Assists in creating and implementing policies and procedures for all clinicians.
→Initiate appropriate preventive and rehabilitative nursing procedures.
→Administer medications and treatments as prescribed by physicians.
→Conduct initial assessments of patients and families to determine home care needs.
→Perform thorough physical assessments and gather comprehensive medical histories.
→Develop and revise individualized plans of care based on patient status and evolving needs.
→Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
→Responsively address inquiries and concerns from patients, caregivers, and medical teams.
→Provide counseling and support to patients and caregivers before, during, and after treatment.
→Supervise home health aides as required.
→Identify discharge planning needs and proactively implement them as part of the care plan development.
