Responsibilities
~1 min read- →Initiate appropriate preventive and rehabilitative nursing procedures.
- →Administer medications and treatments as prescribed by physicians.
- →Conduct initial assessments of patients and families to determine home care needs.
- →Perform thorough physical assessments and gather comprehensive medical histories.
- →Develop and revise individualized plans of care based on patient status and evolving needs.
- →Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
- →Responsively address inquiries and concerns from patients, caregivers, and medical teams.
- →Provide counseling and support to patients and caregivers before, during, and after treatment.
- →Supervise home health aides as required.
- →Identify discharge planning needs and proactively implement them as part of the care plan development.
Requirements

- Preferably 2+ years of nursing experience, with a focus on geriatrics.
- Nursing degree from an accredited institution.
- Excellent bedside manners and exceptional communication skills.
- Working knowledge of healthcare laws and regulations.
- Maintain current BLS CPR certification.
- Possess a valid driver's license and own a reliable, insured vehicle.
