Responsibilities

As a Home Health RN, you will provide skilled nursing care to patients in their homes while working closely with physicians, patients, families, and the interdisciplinary care team.

Your responsibilities will include:

→Conducting comprehensive patient assessments and reassessments
→Developing, implementing, and updating individualized Plans of Care
→Providing skilled nursing interventions based on each patient's clinical needs
→Completing accurate and timely OASIS assessments and clinical documentation
→Educating patients and families regarding disease processes, medications, treatments, and plan of care
→Monitoring patient progress and communicating changes in condition appropriately
→Collaborating with physicians and the interdisciplinary care team
→Promoting patient safety, quality outcomes, and continuity of care
→Providing compassionate, professional care that allows patients to remain safely at home
