Pine Park Health is hiring Physician Assistants (Full-Time) to revolutionize primary care for seniors in the East Bay area.

With dedicated care coordinators handling administrative work and RNs managing triage, you can focus on what you do best: comprehensive clinical care. Our collaborative team structure provides robust support for chart prep, patient education, and follow-up management, creating a workflow that enhances both provider satisfaction and patient outcomes.

Responsibilities

  • Provide comprehensive onsite primary care to seniors in assisted living and senior living communities
  • Develop and execute personalized, patient-centered care plans that address the whole person
  • Partner with our Medical Director to refine clinical policies, protocols, and guidelines as our practice evolves
  • Deliver monthly Chronic Care Management (CCM) visits and oversee CCM documentation
  • Collaborate with senior living leadership and care teams to optimize care plan execution and patient outcomes
  • Clinical credentials: PA certification with a genuine passion for geriatric medicine (prior geriatric experience preferred but not required)

The base salary range for this role is $135,000 to $160,000 plus generous bonus, equity, and benefits. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data. Our compensation structures and ranges are tailored to each geographic zone's unique market conditions to ensure all employees receive fair and competitive compensation based on their roles and locations.
