Position Summary
The Salvation Army Northern Division Anoka 360 Life Center, located in Blaine, Minnesota, provides holistic, person-centered services to individuals and families. Services include food, clothing, utility, rent and housing assistance, case management, spiritual support, and connections to community resources.
The Case Manager provides strength-based, person-centered case management to individuals and families experiencing homelessness or housing instability. This position works with participants to identify needs, navigate housing and community resources, access available assistance, and develop goals that support housing stability and self-sufficiency. The Case Manager also provides direct assistance, completes participant assessments, maintains required documentation, and collaborates with community partners to help individuals and families obtain and maintain stable housing.
$20.81 to $26.67 per hour, 40 hours per week

Essential Functions (approximate time spent)
- Participant Assistance and Housing Support (45%): Provide direct assistance to individuals and families experiencing housing instability. Help participants identify needs, navigate housing and community resources, access available assistance, and address barriers to obtaining or maintaining stable housing.
- Case Management and Participant Support (30%): Provide strength-based, person-centered, holistic case management. Complete participant assessments, develop individualized housing stability plans, coordinate services and referrals, and support participants in achieving goals that promote housing stability and self-sufficiency.
- Documentation and Data Management (10%): Maintain accurate and timely participant case files, database records, and other required program documentation in accordance with organizational requirements.
- Outreach and Community Partnerships (10%): Develop and maintain collaborative relationships with community partners and service providers. Coordinate referrals, connect participants with appropriate resources, and participate in community activities that support participant and program needs.
- Event Support (5%): Assist with seasonal and special events.
Education and Experience
- Bachelor’s degree in a relevant field. An equivalent combination of education and experience may be considered in lieu of a degree.
- Must be licensed by the state board if the degree is in social work.
- Previous experience in social services required.
- Experience providing long-term case management preferred.
- Must pass background checks, possess a valid driver’s license, and meet MVR requirements.

- Excellent organizational skills and attention to detail.
- Proficient with PC, Microsoft Office, databases, and standard office equipment.
Working Conditions
- Primarily sedentary work involving extended periods of sitting and computer use.
- Work is performed in professional office and community-based settings, with regular interaction with participants, community partners, and service providers.
- Requires clear and effective communication with colleagues, participants, and community members.
- Standard business hours with occasional evening and weekend work based on program needs.
- Must occasionally lift up to 20 pounds.
- Position is site-based and not eligible for a hybrid or remote-friendly work arrangement.
Supervisory Responsibilities
- None.
Driving/Travel Requirements
- Regularly operate agency vehicles and travel throughout the service area in support of program operations.
