PAY RATE: $20.67-$24.80 Hourly, Grant Funded Position

JOB SUMMARY
NCI Grant and Per Diem Transition in Place (TIP) program aims to provide community-based case management services using the Critical Time Intervention – Rapid Re-housing (CTI-RRH) model and strength-based case management approach to Veterans and their families enrolled in the Grant and Per Diem Transition in Place (GPD-TIP) program and desire to take action to break the cycle of crisis and vulnerability in their lives and change the trajectory of their lives through this sustainable permanent housing interventions and foster a deeper understanding of the life-changing work carried out by The Salvation Army North and Central Division.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Case Management and Veteran Services (70%)
- Provide intensive, community-based case management services utilizing Critical Time Intervention (CTI), Housing First, Strengths-Based Case Management, Motivational Interviewing, Trauma-Informed Care, and Harm Reduction principles.
- Conduct comprehensive assessments to identify barriers to housing stability, income, employment, health care, behavioral health services, legal issues, and community integration.
- Develop individualized service plans with Veterans and their families that include measurable goals, timelines, and action steps designed to promote permanent housing stability and self-sufficiency.
- Maintain regular contact with enrolled Veterans through office visits, home visits, telephone contacts, video conferencing, and community-based meetings as appropriate.
- Conduct outreach, engagement, intake, enrollment, and orientation activities for eligible Veterans and their households.
- Assist Veterans in locating, securing, and maintaining safe and affordable permanent housing.
- Facilitate housing searches, coordinate housing inspections, and assist with lease-up activities consistent with VA requirements and program guidelines.
- Provide housing stabilization services including landlord mediation, tenancy support, conflict resolution and crisis intervention.
- Conduct home visits and welfare checks to ensure participant safety, housing quality, and continued program engagement.
- Connect Veterans and family members with community resources, including employment services, behavioral health treatment, primary medical care, public benefits, education, transportation, childcare, legal services, and financial assistance programs.
- Advocate on behalf of Veterans with landlords, employers, service providers, Veterans Affairs staff, courts, and community agencies.
- Facilitate life-skills education, financial literacy, budgeting, tenant education, resiliency training, and other skill-building activities that support long-term housing retention.
- Assist Veterans in developing income sources, accessing benefits, and increasing financial stability through coordination with appropriate local, state, federal, and VA resources.
- Develop discharge plans that support sustainable housing outcomes and coordinate warm handoffs to community providers when participants exit the program.
- Documentation, Compliance, and Administrative Expectations (20%)
- Maintain accurate, timely, and complete participant records in HMIS/SIMS and any other required electronic data systems.
- Complete all program documentation, assessments, housing plans, service plans, case notes, inspections, referrals, discharge records, and outcome measurements in accordance with VA, grant and Salvation Army requirements.
- Enter case notes and service transactions within established program timelines and quality standards.
- Track participant progress toward goals and regularly update service plans based on participant needs and outcomes.
- Assist with collection and documentation of program fees and other financial transactions as required by program guidelines.
- Coordinate utility transfers, housing-related financial requests, inspections, and supporting documentation.
- Ensure confidentiality and secure management of participant information consistent with federal regulations, grant requirements, and agency policies.
- Maintain compliance with all VA Grant and Per Diem (GPD), Transition in Place (TIP), HMIS and Salvation Army policies, procedures, Code of Ethics, and contractual requirements.
- Successfully satisfy the training requirements associated to the position and as required by The Salvation Army.
- Community Collaboration and Professional Development (10%)
- Participate in Coordinated Entry meetings, By-Name List case conferencing, Continuum of Care activities, community planning meetings, and Veteran service coalitions.
- Build and maintain collaborative relationships with VA Medical Centers, community agencies, landlords, housing providers, and Veteran-serving organizations.
- Attend Stand Downs, resource fairs, outreach events, and community engagement activities to identify and assist Veterans experiencing homelessness.
- Participate in staff meetings, supervision sessions, trainings, conferences, and professional development activities.
- Maintain required certifications, including Safe From Harm compliance and any position-specific training requirements.
- Promote the mission, values, and service philosophy of The Salvation Army in all interactions with participants, staff, volunteers, and community partners.
Travel
- Involves driving to offsite meetings, conducting Veteran home visits and coordinating Department of Veterans Affairs GPD inspections.
- May be required to attend mandatory meeting events located throughout central Illinois.
- Must be willing and able to drive within the service area of Union, Johnson, Jackson, Williamson, Pope, Massac, Saline, Franklin, Pulaski, Perry, Hamilton and Gallatin Counties.

MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES
Education:
- Bachelor’s degree in social work, public health and/or human services
OR
- Experience working with the homeless population and/or working with Veterans.
- An in-depth knowledge of VA programs and guidelines
- And/or Veteran status
Experience:
- Experience working with the homeless population and/or working with Veterans.
- Minimum of two (2) years of experience providing direct case management, care coordination, housing stabilization, homeless services, behavioral health services or related human services support.
- Experience working with vulnerable populations experiencing homelessness, housing instability, poverty, mental health challenges, substance use disorders or related barriers.
- Experience maintaining accurate electronic case records and timely case notes.
- Experience conducting client assessments, developing service plans, documenting participant progress, and coordinating referrals to community resources.
- Experience working independently while managing multiple participant cases and competing priorities.
- Preferred experience utilizing Critical Time Intervention (CTI), Strengths-Based Case Management, Motivational Interviewing, Harm Reduction, Trauma-Informed Care, or Recovery-Oriented approaches.

JOB LIMITATIONS
- This individual will only commit resources of The Salvation Army that have been allocated or approved.
- This individual will adhere to all policies and procedures in carrying out the responsibilities of this position. The individual will keep the designated Supervisor informed of all critical issues relating to their area of responsibility.
PHYSICAL DEMANDS/WORK ENVIRONMENT
- This position may be required to do light to moderate physical work. Sometimes heavier.
- Must be able to lift up to 30 pounds.
- To successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment including a PC and potentially make public presentations/interactions.
- Adept with Microsoft Office, Word, Outlook, Excel and PowerPoint.
- The work environment for this position includes an office environment with a low to moderate noise level.
- Must be able to travel to multiple sites.
- Able to accurately type 45 words per minute.
- This individual must be able to travel to meet the needs of the position. A valid MVR is required.
REPORTING RELATIONSHIPS
- This position reports to the GPD-TIP Program Manager.
- In context related to this position’s duties, this individual will interact with a wide range of contacts both internal and external. In these contacts he/she will act as a representative of The Salvation Army and its mission.

- This individual will be evaluated on how effectively the outcomes of this position are achieved. Timeliness and accuracy of accomplishing assigned goals will be reviewed. An evaluation of how effective service is provided. Performance will also be determined by outcome information provided during weekly staff meetings and monthly reports.
