JOB SUMMARY
NCI Supportive Services for Veteran Families (SSVF) program aims to provide critical time intervention, strength-based case management approach to Veterans and their families with a desire to take action to break the cycle of crisis and vulnerability in their lives and change the trajectory of their lives through sustainable permanent housing interventions and foster a deeper understanding of the life-changing work carried out by The Salvation Army North and Central Division.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Case Management (70%)
- Ensure program criteria, referrals and procedures are maintained for traditional and Shallow Subsidy SSVF program Veteran participants.
- Provide intensive case management services utilizing Critical Time Intervention (CTI), Housing First, Strength-Based Case Management, Motivational Interviewing, Trauma-Informed Care, and Harm Reduction principles.
- Develop individualized service plan with the participant within one week of enrollment with specific measurable, short- and long- term goals, timelines, and action steps designed to promote housing stability and self-sufficiency.
- Maintain regular contact with enrolled Veterans through office visits, home visits, telephone contacts, video conferencing, and community-based meetings as appropriate.
- Conduct comprehensive assessments to identify barriers to housing stability, income, employment, health care, behavioral health services, legal issues, and community integration.
- Provide housing stabilization searches including landlord mediation, tenancy support, landlord/tenant conflict resolution and crisis intervention.
- Conduct home visits and welfare checks to ensure participant safety, housing quality and continued program engagement.
- Assist Veterans in locating, securing, and maintaining safe and affordable permanent housing and conducting housing inspections.
- Provide intake including required documentation for admission, referral needs, program details and expectations.
- Conduct outreach, engagement, intake, and enrollment activities for eligible Veterans and their households.
- Assist Veterans in filing for SSDI/SSI benefits through SSA’s SOAR certification program.
- Using strengths-based perspective, encourage positive lifestyle changes for Veterans to maintain housing.
- Connect Veterans and family members with community resources, including employment services, behavioral health treatment, primary medical care, public benefits, education, transportation, childcare, legal services and financial assistance programs.
- Flex schedule to meet with full time employed Veterans during non-standard business hours.
- Advocate on behalf of Veterans with landlords, employers, service providers, Veterans Affairs staff, courts, and community agencies.
- Manage a case load of up to 15 Veteran households in Shallow Subsidy for up to 24 months, in addition to households enrolled in traditional SSVF.
- Implements the SSVF Shallow Subsidy Initiative in collaboration with the traditional SSVF program.
- Ensure client confidentiality in accordance with established procedures and regulations.
- Process Temporary Financial Assistance for up to 24 months for each Veteran enrolled in Shallow Subsidy.
- Process Temporary Financial Assistance for Veterans enrolled in traditional SSVF in compliance with SSVF regulations.
- Performs other duties as assigned as related to the mission and goals of the SSVF Program.
- Administrative Responsibilities (20%)
- Maintain complete and accurate electronic client records that includes the assessment, eligibility, intake, case plan, documentation of progress, challenges, accomplishments, discharge plan and all follow up contact.
- Perform HMIS (SIMS) data entry, review data for accuracy, completeness and comprehensiveness.
- Provide electronic detailed case note records.
- Participate in quality assurance/CARF Task Force Group.
- Comply with all policies and procedures of the program and the Commission on Accreditation of Rehabilitation Facilities (CARF).
- Collaboration (10%)
- Attend community meetings and Coordinated Entry By-Name-List reviews.
- Attend community Veteran events such as Stand Down, Veteran Resource Fairs, etc.
- Work in partnership with other SSVF Case Managers, Intake Specialists, Outreach Workers, Health Care Navigators and Legal Services benefits staff.
- Communicate effectively with VA HUD-VASH and Salvation Army personnel.
- Develop and maintain relations with local social services, public benefit agencies, faith-based and community-based organizations, VA facilities, Vet Centers and Veterans organizations.
- Attend weekly, monthly meetings and annual training events.
TRAVEL
- Travel throughout the program’s service area is required on a regular basis to meeting with Veterans, community partners and to attend mandatory meeting events.
Administrative Expectations
- Successfully satisfy the training requirements associated to the position and as required by The Salvation Army.

MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES
Education:
- Bachelor’s degree in human services, social work, or a closely related field
OR
- An in-depth knowledge of VA programs and guidelines
- And/or Veteran status
Experience:
- One year of direct, full-time case management experience working with the homeless population and/or working with Veterans
- Valid Driver’s license/MVR clearance
Skills/Abilities
- Team player, self-starter, and perform well with minimal supervision.
- Must have an interest and ability to work with people of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner.
- Must be able to work in diverse environments such as homeless shelters, service facilities, streets, offices, hospitals and all other locations as necessary to fulfill program objectives.
- Experience in working with Veteran population and have experience in areas of substance abuse, Veterans assistance and PTSD is beneficial.
- Prior knowledge and training in Critical Time Intervention (CTI), harm reduction, Housing First, Trauma Informed Care, motivational interviewing and conflict resolution is preferred.
- Excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member.
- Proficiency in computer software applications, including demonstrated experience in using email, MS Office applications, internet, and social media platforms.
- Must be able to work days, evenings, weekends and holidays as necessary. Especially in emergent situations.

JOB LIMITATIONS
- This individual will only commit resources of The Salvation Army that have been allocated or approved.
- This individual will adhere to all policies and procedures in carrying out the responsibilities of this position. The individual will keep the designated Supervisor informed of all critical issues relating to their area of responsibility.
PHYSICAL DEMANDS/WORK ENVIRONMENT
- This position is required to do light physical work.
- In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and a PC.
- Adept with Microsoft Office, Word, Outlook, Excel and PowerPoint.
- Must be able to lift or move up to 30 lbs.
- The work environment for this position normally includes an office environment with a low to moderate noise level.
- Must be able to travel to multiple sites.
- Able to accurately type 45 words per minute.

- This position reports to the SSVF Region 3 Supervisor.
- In context related to this position’s duties, this individual will interact with a wide range of contacts both internal and external. In these contacts he/she will act as a representative of The Salvation Army and its mission.

- This individual will be evaluated on how effectively the outcomes of this position are achieved. Timeliness and accuracy of accomplishing assigned goals will be reviewed. An evaluation of how effective service is provided. Performance will also be determined by outcome information provided during weekly staff meetings and monthly reports.
