The Hospital Navigator serves as the primary liaison between hospital stakeholders, patients, and Superior Mobile Health operations teams to coordinate safe, compliant, and efficient patient transportation services. This role works closely with hospital leadership, case management, discharge planning, and emergency department teams to facilitate transport requests, obtain required documentation and authorizations, and ensure patients receive the appropriate level of transportation.
The Hospital Navigator also monitors transportation performance, resolves service concerns, and provides reporting and insights to health system partners to support operational excellence and patient access to care.
Responsibilities
~1 min read- Serve as the primary liaison between hospital stakeholders, patients, and Superior Mobile Health personnel
- Coordinate transportation requests and ensure the appropriate level of service is scheduled
- Obtain and verify patient information, required documentation, authorizations, and insurance information
- Maintain knowledge of ambulance medical necessity requirements and transportation compliance standards
- Develop and maintain strong working relationships with hospital leadership, case management teams, emergency department staff, and operational partners
- Conduct regular on-site visits to support communication, collaboration, and service alignment
- Resolve transportation-related concerns in a timely and professional manner
- Prepare and present transportation utilization, operational, and financial reports
- Monitor service trends and identify opportunities for process improvement
- Ensure accurate data entry and documentation within company systems
- Comply with all company policies and procedures
- Perform other duties as assigned
Requirements
~1 min read- EMS, case management, patient access, transfer center, or care coordination experience
- Experience with interfacility transport coordination
- Familiarity with PCS forms, transfer packets, and transport documentation
- Associate's or Bachelor's degree in a healthcare-related field
- EMT, Paramedic, LVN, RN, or other clinical certification
- BLS/CPR Certification
- Ability to lift up to 25 pounds occasionally
- Ability to effectively communicate verbally and in writing
- Ability to use a computer and mobile technology devices
- Ability to sit, stand, walk, and travel throughout the workday
- Healthcare operations, patient flow, discharge planning, EMS, or hospital transport coordination experience
- Experience working with clinical staff in a fast-paced healthcare environment
- Understanding of medical necessity requirements and transport coordination processes
- Knowledge of HIPAA and patient privacy requirements
- Ability to work independently while representing the organization professionally
- High School Diploma or GED
