About the Role

We are seeking a Bilingual Clinical Care Coordinator with a strong clinical background to remotely support a U.S.-based healthcare practice. This role is ideal for a qualified healthcare professional who has transitioned into healthcare administration, care coordination, or virtual medical support.

The Clinical Care Coordinator will serve as an important link between patients, providers, insurance companies, and other members of the care team, helping ensure patients receive timely and well-coordinated care.

Responsibilities

  • →Communicate with patients regarding appointments, treatment plans, follow-ups, referrals, and other care needs.
  • →Coordinate patient care between providers, specialists, facilities, pharmacies, and other healthcare professionals.
  • →Assist with patient intake and maintain accurate patient information within the EHR/EMR.
  • →Document patient interactions, clinical information, and provider instructions accurately.
  • →Coordinate and follow up on referrals and specialist appointments.
  • →Assist with insurance verification, eligibility checks, and prior authorizations.
  • →Follow up on outstanding medical records, laboratory results, imaging, referrals, and other clinical documentation.
  • →Conduct patient follow-up and care management calls, including Chronic Care Management (CCM) calls when required.
  • →Review patient information and identify issues that require escalation to the appropriate clinical provider.
  • →Communicate provider notes, instructions, and other relevant information to patients.
  • →Support prescription/refill requests and other clinical administrative workflows as appropriate.
  • →Maintain accurate, confidential, and HIPAA-compliant patient records.
  • →Provide compassionate and professional patient support while helping patients navigate their care journey.

Requirements

  • Clinical background required; candidates with training or professional experience as a Nurse, Doctor, Physician Assistant, Medical Assistant, or other healthcare professional are encouraged to apply.
  • Strong understanding of medical terminology and clinical workflows.
  • Previous experience in care coordination, patient coordination, medical virtual assistance, clinical administration, or a similar healthcare role is strongly preferred.
  • Experience supporting U.S.-based healthcare practices is highly preferred.
  • Familiarity with EHR/EMR systems.
  • Experience with insurance verification, prior authorizations, referrals, or care management is an advantage.
  • Strong English communication skills; bilingual English/Spanish candidates are highly preferred.
  • Excellent patient communication, organization, documentation, and follow-up skills.
  • Comfortable communicating with patients, healthcare providers, insurance companies, and other members of the care team.
  • Ability to work independently in a remote environment while managing multiple patient needs and priorities.
  • Understanding of HIPAA and patient confidentiality requirements.

They understand medical terminology and clinical workflows but are equally comfortable coordinating appointments, following up on referrals and authorizations, documenting in an EHR, communicating with patients, and ensuring that important aspects of a patient's care do not fall through the cracks.
