• →Cares for Suvida Healthcare’s patients and focuses on outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention 
• →Manages patients in a multidisciplinary value-based care model 
• →Serves as the leader for their respective care team within the Neighborhood Center 
• →Collaborates effectively with Suvida Healthcare’s multidisciplinary care team 
• →Focuses on providing 5-Star care and strives to provide a high level of customer service 
• →Keeps a medical history and medical records 
• →Refers the patient to specialists as needed 
• →Attends daily morning huddle with care team to discuss patients to be seen that day and works to close any potential care gaps 
• →Attends weekly huddle with multidisciplinary care team to review and manage high-risk patients within the care team 
• →Begins to influence department’s strategy 
• →Makes decisions on moderately complex to complex issues regarding technical approach for project components, and work is performed without direction 
• →Exercises considerable latitude in determining objectives and approaches to assignments 
• →Oversees Advanced Practice Providers as needed within their Neighborhood Center 
• →Reports to the Neighborhood Center Medical Director 

• Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) 
• Board certification or eligibility in Family or Internal Medicine 
• Current DEA license
