About the Role
~1 min readTandem Care is hiring a Care Coordinator who thrives in fast-paced, high-volume home care environments and has real experience working directly with caregivers and clients.
This role sits at the intersection of client experience, caregiver support, and care planning. You’ll be on the phone, in systems, and communicating constantly — reviewing care needs, coaching caregivers, addressing issues as they come up, and making sure care plans actually work in the real world.
This is not a passive, intake-only, or administrative role. If you’re energized by problem-solving, juggling multiple clients at once, and making judgment calls throughout the day, this role may be a great fit.

-
Are detail-oriented and decisive, not just empathetic

Understand that in home care, things change — and follow-up matters
Requirements
