The University of Miami/UHealth Department of Medicine  has an exciting opportunity for a Full-Time Nurse Navigator 1 to work in the UHealth Campus. 

 

The Rheumatology Nurse Navigator provides proactive clinical navigation and coordination for new and complex rheumatology patients throughout the care continuum. The role focuses on pre-visit clinical chart preparation, identification of barriers to care, patient education, complex multidisciplinary coordination, and closed-loop follow-through. The Nurse Navigator serves as the day-to-day workflow lead for the Patient Navigators and collaborates with physicians, the APRN, Registered Nurses, CBOR staff, pharmacies, infusion services, and internal and external healthcare partners. Clinical direction and escalation are provided by the APRN and physicians; administrative matters such as payroll, HR, and staffing remain under the Division Administrator.

Responsibilities

  • →Performs clinical chart preparation for new rheumatology patients by reviewing available referral documentation, outside medical records, laboratory results, imaging, prior specialty evaluations, medication history, and other relevant clinical information before the initial visit.
  • →Identifies missing records, laboratory results, imaging, prior treatment information, or other documentation needed for an effective rheumatology evaluation and directs Patient Navigators to obtain outstanding information.
  • →Reviews available referral and clinical information to identify patients who may require additional clinical review, expedited attention, or coordination before the scheduled visit, and escalates concerns to the APRN or physician as appropriate.
  • →Assesses barriers to care and coordinates appropriate resources to support timely access, continuity, and completion of the treatment plan.
  • →Provides high-level care coordination for complex rheumatology patients, particularly those requiring multidisciplinary or cross-specialty management.
  • →Serves as a clinical liaison among patients, caregivers, rheumatology providers, primary care providers, external specialists, hospitals, facilities, and other departments involved in the patient's care.
  • →Provides patient and caregiver education, reinforces provider-directed treatment plans, and helps patients understand next steps, testing, referrals, and follow-up needs.
  • →Coordinates continuity of care for patients requiring outside records, specialty services, multiple healthcare resources, or complex follow-up.
  • →Leads the day-to-day workflow of the two Patient Navigators, including prioritization and assignment of record retrieval, PCP/facility outreach, routine patient follow-up, and documentation needs.
  • →Reviews unresolved or escalated Patient Navigator cases and determines whether the issue requires Nurse Navigator intervention, Registered Nurse clinical intervention, or APRN/physician review.
  • →Collaborates with the RNs to ensure clinical issues identified during navigation or chart preparation are appropriately routed and resolved without transferring routine back-end nursing work to the navigation function.
  • →Collaborates with the APRN and physicians regarding complex clinical coordination needs and escalates clinical concerns requiring advanced-practice or physician decision-making.
  • →Monitors navigation worklists and outstanding action items to promote closed-loop resolution before or after the patient's appointment, as appropriate.
  • →Supports quality-improvement activities related to new-patient readiness, access, continuity of care, patient experience, and navigation outcomes.
  • →Serves as a clinical resource to Patient Navigators, CBORs, and administrative staff regarding appropriate routing of patient-care needs.
  • →Maintains professional knowledge through continuing education and adheres to University and unit-level policies, procedures, documentation standards, and patient confidentiality requirements.

Requirements

Valid State of Florida Registered Nurse (RN) license

Minimum 2 years of relevant work experience
