  • Connect people with services, communities and opportunities
  • SCHADS Level 3 ($40.49 - $43.42 per hr) + Leave Loading + Salary Packaging
  • Work alongside GPs and local services to create better outcomes  

What if your work could help someone feel more connected, supported and confident to navigate the world around them?

The role is also about building connections between services by working closely with GPs, other health professionals, community organisations, families and carers to create coordinated pathways and reduce the barriers people can face when trying to access support.

  • Experience working with people experiencing mental health or psychosocial challenges
  • A genuine commitment to person-centred, recovery-oriented practice
  • The ability to listen, understand and work alongside people as the experts in their own lives
  • Confidence navigating health, community and social service systems
  • A collaborative approach to working with multidisciplinary teams and external stakeholders
  • Strong organisation and documentation skills
  • The ability to build community networks and identify opportunities that reduce isolation and increase participation

​You will need:

  • Tertiary qualification in Social Work, Mental Health, Nursing, Allied Health, Aboriginal Health, Community Services or a related field (completed or in progress), or equivalent relevant experience in the sector
  • Strong understanding of recovery-oriented, trauma-informed and person-led practice
  • Right to work in Australia 
  • 100 points of ID 
  • Valid Driver Licence and willingness to travel
  • Satisfactory National Police Record Check 
  • International Police Check (if appliable) 
  • Current Working with Children Check and NDIS Worker Screening Check  
  • NDIS Worker Orientation Module Certificate – Quality Safety and You
