Responsibilities
~2 min read- →Establishes the enterprise vision, strategic direction, governance framework, workforce standards, and performance expectations for the Community Health Worker function, ensuring alignment with organizational priorities, enterprise social health strategy, and future-state care delivery models.
- →Selects, coaches, and develops state-based Community Health Worker leadership teams, including managers and other leaders responsible for supervisors, team leads, and Community Health Workers. Establishes leadership expectations, fosters succession planning, develops future leaders, and builds a high-performing culture focused on accountability, engagement, operational excellence, and measurable outcomes.
- →Provides strategic and operational oversight for enterprise Community Health Worker services across multiple states, including workforce planning, staffing methodologies, deployment strategies, field operations, productivity expectations, service quality, care model alignment, and integration with regional and enterprise priorities.
- →Leads the development, governance, implementation, and continuous advancement of enterprise Community Health Worker operating models, workflows, workforce standards, and program structures that support consistent, scalable, culturally responsive, and high-quality community-based services.
- →Establishes and monitors key performance indicators, program evaluation methods, reporting approaches, and data-informed improvement plans to measure CHW productivity, outcomes, patient impact, community benefit, quality, and operational effectiveness.
- →Builds and sustains strategic partnerships with executive leaders, physicians, market leadership, operational partners, Social Health Operations, Finance, Human Resources, Information Technology, Marketing, Communications, analytics teams, and other stakeholders to advance enterprise Community Health Worker priorities and organizational objectives.
- →Builds and sustains strategic partnerships with community organizations, public officials, health systems, policy leaders, philanthropic organizations, corporate partners, foundations, advocacy organizations, and other external stakeholders to strengthen community capacity, improve access to health and social care services, and advance enterprise community health priorities.
- →Develops, recommends, and oversees operating and capital budgets, staffing investments, workforce expansion strategies, resource allocation, and funding priorities for the enterprise Community Health Worker function while ensuring responsible financial stewardship and long-term program sustainability.
- →Represents the enterprise Community Health Worker workforce in executive leadership forums, strategic planning initiatives, governance committees, community collaborations, and cross-functional enterprise initiatives. Serves as the executive subject matter expert for Community Health Worker strategy, influences organizational alignment, and advances innovative care models that improve population health, social health, value-based care, and community outcomes.
- →Establishes enterprise governance structures that promote consistent Community Health Worker practice standards, workforce accountability, leadership development, operational excellence, and standardized implementation across all markets and states.

- Certified Community Health Worker (where applicable), Lean Six Sigma Green Belt or higher, Project Management Professional (PMP), Change Management certification (e.g., Prosci), Population Health, Community Health, or Public Health leadership certification preferred, or related field.
- Bachelor's degree in Public Health, Social Work, Community Health, Healthcare Administration, Human Services, Nursing, Business Administration, or a related field, or equivalent knowledge typically acquired through a combination of education, training, and progressive professional experience.
- Master's degree in Public Health, Social Work, Healthcare Administration, Business Administration, Community Health, Population Health, Organizational Leadership, or a related field preferred.
- Seven or more years of progressive experience in community health, social health, population health, healthcare operations, public health, community-based programs, care coordination, health equity, or related healthcare leadership roles.
- Minimum of five years of leadership experience with responsibility for workforce management, program operations, organizational strategy, performance accountability, and cross-functional leadership.
- Experience leading large, geographically dispersed teams through managers and/or supervisors within a complex, matrixed healthcare organization.
- Experience leading enterprise or multi-state Community Health Worker workforce operations, organizational design, workforce planning, staffing optimization, and scalable operational growth.
- Experience leading enterprise initiatives involving workforce transformation, operational standardization, performance improvement, community partnership development, and implementation of strategic organizational priorities.
- Experience leading enterprise, regional, or multi-state Community Health Worker, social health, community health, population health, value-based care, or community benefit programs within an integrated healthcare system.
- Experience managing leaders responsible for large frontline workforces across multiple markets, states, or service areas.
- Experience with Epic, social care technology platforms, workforce analytics, performance dashboards, grant-funded programs, alternate payment models, regulatory requirements, and community partnership strategy.
- Experience developing enterprise leadership structures, succession planning strategies, and workforce development initiatives that support long-term organizational growth.
- Knowledge of Community Health Worker workforce strategy, community health, social health, population health, health equity, healthcare operations, workforce governance, care coordination, patient navigation, community partnerships, workforce planning, performance management, organizational development, healthcare technology platforms, electronic medical records, and data-driven operational improvement.
- Strong strategic planning, operational leadership, workforce development, executive communication, facilitation, stakeholder influence, collaboration, organizational change management, financial stewardship, performance management, and leadership development skills.
- Ability to establish enterprise vision for the Community Health Worker workforce; develop and lead high-performing leadership teams; govern enterprise workforce models; influence executive and operational leaders; manage competing strategic priorities; build collaborative partnerships across clinical, operational, and community stakeholders; and drive measurable workforce and operational performance within a complex, matrixed healthcare organization.
- Expertise in enterprise Community Health Worker workforce strategy, organizational design, workforce governance, leadership development, health equity, community partnerships, value-based care, population health, performance analytics, continuous improvement, and enterprise operational transformation.
- Advanced ability to influence executive leadership, establish enterprise governance structures, lead workforce transformation initiatives, align diverse stakeholder groups, develop future leaders, drive organizational change, and scale Community Health Worker workforce models across a large, integrated healthcare system.

Requirements

Requires prolonged periods of sitting, computer use, virtual meeting participation, telephone communication, and frequent interaction with leaders, team members, community partners, and stakeholders. Requires travel across markets, states, facilities, and community locations and may involve exposure to weather and road conditions. Must be able to operate equipment necessary to perform the job and may occasionally lift or move up to 20 to 30 pounds to complete work-related tasks.
