One Health currently operates 35 primary care offices across the greater Charlotte and Winston-Salem areas of North Carolina. Our specialties include Family Medicine, Internal Medicine, Pediatrics, Sports Medicine, Rheumatology, OB/GYN, and Urgent Care.

We are seeking a dedicated Nurse Practitioner or Physician Assistant to join the Family Medicine team at Atrium Health One Health Mount Island. This position will provide high-quality, comprehensive primary care during extended weekend hours, expanding access and providing greater flexibility for patients and families in the Mount Island community.


This unique three-day schedule offers the opportunity to provide full-spectrum family medicine while helping One Health expand convenient access to primary care.


Responsibilities

  • →Provide comprehensive, patient-centered family medicine care across a broad spectrum of ages, disease states, and chronic conditions.
  • →Evaluate, diagnose, and treat acute illnesses and injuries within the scope of outpatient family medicine.
  • →Manage chronic conditions and provide preventive care, health screenings, and patient education.
  • →Develop individualized treatment plans using evidence-based clinical practices.
  • →Coordinate care with physicians, specialists, clinical team members, and other healthcare professionals as appropriate.
  • →Support continuity of care through appropriate follow-up, referrals, and care coordination.
  • →Utilize One Health and Advocate Health resources, technology, and clinical support services to enhance patient care.
  • →Contribute to a collaborative practice environment that values clinical expertise, teamwork, and an exceptional patient experience.
