The Advanced Practice Provider (APP) in Transplant Nephrology provides comprehensive care to patients undergoing evaluation for kidney transplantation as well as those who are status post–kidney transplant. This role includes inpatient rounding and outpatient clinic responsibilities, working collaboratively within a multidisciplinary transplant team to deliver high-quality, patient-centered care. The APP will work five (5) days per week with eight (8) hour shifts.

Responsibilities
~1 min read- →Daily rounding on hospitalized kidney transplant recipients and transplant evaluation patients
- →Assessment, diagnosis, and management of post-transplant medical conditions
- →Monitoring of renal allograft function, laboratory results, and imaging - Documentation in the electronic medical record (EMR)
- →Collaboration with transplant nephrologists, surgeons, pharmacists, nursing, and care coordination teams
- →Participation in discharge planning and patient education
- →Evaluation and management of post–kidney transplant patients
- →Pre-transplant patient evaluations and medical risk assessments
- →Ordering and interpretation of diagnostic tests and laboratory studies
- →Management of immunosuppressive therapies under protocol and physician supervision
- →Longitudinal follow-up care and coordination with referring providers
- Serves as an active member of Transplant Selection Committees, contributing to candidate evaluation, risk stratification, and multidisciplinary decision-making in accordance with OPTN/UNOS and institutional guidelines.
- Participates in Quality Assurance and Performance Improvement (QAPI) committees, reviewing transplant outcomes, identifying trends, and supporting initiatives to improve patient safety, regulatory compliance, and clinical excellence.
- Engages in Population Management and Outcomes Review Committees, assisting with systematic review of pre- and post-transplant populations, graft outcomes, readmissions, and long-term care metrics.
- Contributes to Morbidity and Mortality (M&M) conferences and case reviews as appropriate.
- Adult patients who are post–kidney transplant
- Adult patients undergoing pre-evaluation for kidney transplantation
- Qualifications – Required
- Graduate of an accredited Nurse Practitioner or Physician Assistant program - Active, unrestricted APP license in the state of practice - National certification (AANP/ANCC for NP or NCCPA for PA)
- Eligibility for DEA licensure
- Strong clinical, communication, and documentation skills
