We're looking for a passionate nurse to join our team as a Clinical Supervision Nurse (LPN). This role will report into the Director of Nursing in Georgia. This is a Full-Time opportunity based in the Atlanta, GA.
Responsibilities
~1 min readConduct face-to-face supervisory visits in the member's home every 14 days (complying with the Georgia DCH requirement of at least every 20 days/biweekly).
Observe the paid family caregiver actively performing delegated personal care tasks (bathing, grooming, transfers, and medically modified feeding) to ensure safe, infection-free, and anatomically correct techniques are utilized.
Verify that the home environment remains clean, safe, and stocked with necessary medical and emergency supplies.
Perform targeted, pediatric-focused clinical observations of the member during each visit (e.g., skin integrity checks, basic respiratory effort, and overall physical status).
Promptly escalate any change in the member's medical status, hospitalizations, or modifications to medical equipment (e.g., changes to G-tube sizes, ventilator settings, or medication regimes) directly to the supervising RN Case Manager.
Document any regression or progression in the member's functional abilities that may require a modification to GAPP authorized hours.
Review and reinforce the caregiver teaching plan, verifying that the family caregiver remains competent in both routine ADL support and emergency protocols (e.g., seizure rescue, basic life support, G-tube dislodgement).
Document ongoing caregiver competencies using the agency’s GAPP Skills Checklist.
Audit the home-bound GAPP Daily Activity Checklists/Service Records.
Ensure the caregiver and member representative are initialing daily and signing weekly/biweekly, checking for the required "task-times-frequency" alignment.
Verify that clinical notes and care logs are free of conflicting data (e.g., ensuring documented care hours perfectly match authorized GAPP PCSS schedules to prevent PA billing delays).
Submit detailed, objective supervisory visit notes to the agency portal immediately following each visit.
Collaborate closely with the supervising RN to ensure all LPN supervisory documentation is co-signed and dated by the RN within the mandated 10-business-day window.
Requirements
~1 min readActive, unencumbered Licensed Practical Nurse (LPN) license in the State of Georgia.
Minimum of 1–2 years of pediatric nursing experience; prior experience in home health, GAPP, or Medicaid waiver programs is highly preferred.
Familiarity with the Georgia Department of Community Health (DCH) GAPP Medicaid Provider Manual guidelines.
Exceptional clinical observation, documentation accuracy, and clear interpersonal communication skills (necessary for coaching and training family caregivers).
Reliable transportation and willingness to travel within the designated regional territory for home visits.
