Responsibilities
~1 min read- →Respond quickly to hospice referrals and complete real-time bedside clinical assessments
- →Support the admission process by explaining hospice services and benefits to patients and families in both home and clinical settings
- →Gather comprehensive physical, emotional, social, and spiritual information to determine patient needs
- →Conduct clinical assessments to support physician determination of hospice eligibility
- →Build and maintain strong relationships with patients, families, and referral partners
- →Address customer needs and concerns promptly and professionally
- →Complete all required admission documentation, including consents and CTI coordination
- →Facilitate and activate the admission workflow for eligible patients
- →Coordinate General Inpatient (GIP) admissions with hospice centers
- →Serve as an on-site clinical resource for immediate admissions or to support field RNs
- →Collaborate with Transitional Care Navigators, physicians, and hospital case managers to ensure smooth transitions
- →Educate hospital staff on hospice eligibility, referral processes, and available services
- →Participate in weekend or after-hours admission support as needed
- →Perform other duties as assigned
- Graduate of an accredited registered nursing program
- Current RN license in the state of practice
- Current CPR and First Aid certification
- At least 1 year of experience (hospice experience preferred)
- Willingness to travel 50% or more within assigned territory
- Strong clinical judgment and ability to make independent decisions
- Excellent communication skills and the ability to support patients and families with compassion
- Knowledge of quality assessment and assurance practices
- Valid driver’s license and proof of insurance
