Ambercare Hospice is seeking a compassionate and detail-oriented Weekend Intake Coordinator to join our team in Albuquerque, NM. This role is an opportunity to serve as a vital connection point for patients, families, referral sources, and clinical teams during the hospice admission process.

As a Weekend Intake Coordinator, you will help facilitate timely and seamless admissions, ensuring patients receive the care and support they need when they need it most.     Friday: 2:00 PM to 10:00 PM
    Saturday: 12-hour shift
    Sunday: 12-hour shift
    Monday: Coverage through approximately 10:00 AM to 11:00 AM

What You’ll Do:

  • Receive referrals by phone or in person for designated department
  • Provide an explanation of the benefits to patients, families, and community contacts
  • Assist with finding alternative services when the patient is not a candidate for hospice
  • Oversee scheduling of initial nursing assessment and admission
  • Maintain admission packets and ensure that proper forms are available to admissions staff
  • Assist community outreach representatives with marketing activities to promote relations with referral sources
  • Assist with the completion of admission paperwork as needed
  • Adhere to work schedule and maintain good punctuality and attendance record
  • Provide telephone and clerical support to other staff members as time allows
  • Maintains accurate clinical records, keeping track of goals and progress of clients’

Qualifications:

  • Be eighteen (18) years of age or older
  • At least one (1) year experience in homecare, hospice, and/or healthcare-related field preferred
  • Ability to complete necessary documentation appropriately, legibly, and timely
  • Ability to handle stressful and delicate situations
  • Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
  • Valid driver’s license and proof of insurance are required
