Abstracts clinical information from medical records for Inpatient and Outpatient Core Measure submission to CMS and Joint Commission. Conducts chart abstraction in a timely manner to maintain compliance with external regulatory agencies. Processes and maintains data according to established procedures outlined in the National Hospital Quality Measure Guidelines. Ensures data collection and entry is timely, thorough, and accurate. Collaborates with frontline staff, department directors, and medical staff to ensure accurate data abstraction. Provides education to frontline staff, including physicians, regarding Core Measure requirements and performance. Attends and leads Evidence Based Medicine Core Measure Service Line Team meetings. Assists with Core Measure data analysis and evaluates opportunities for improvement. Assists Core Measure teams in preparing Performance Improvement graphs and CRAF analysis. Performs other duties as assigned. Distributes data in usable formats, such as Excel spreadsheets, as directed. Prepares and presents reports utilizing statistical and graphic software to Leadership and Medical Staff.Knowledge, Skills, and Abilities:
• Computer, telephone, fax machine, copier, printer, etc. [Required]

Education:
• Bachelor's of Nursing [Preferred]

Field of Study:
• Graduate of an Accredited School of Nursing
• BSN

Work Experience:
• 3+ nursing experience with demonstrated qualities of leadership and organizational ability. [Preferred]
• Previous experience in Performance Improvement preferred [Preferred]
