Responsibilities:
- Provides leadership and oversight of daily clinical operations within the Skilled Nursing Facility.
- Maintains a visible presence on resident care units and serves as a resource to Nurse Managers, Supervisors, RNs, LPNs, CNAs, and other clinical Care Members.
- Collaborates with Nursing leadership to ensure appropriate delivery and coordination of resident care.
- Assists with identifying and resolving clinical and operational concerns that may affect resident safety, quality of care, staffing, or workflow.
- Participates in daily clinical meetings, interdisciplinary rounds, resident-care conferences, and other operational meetings as assigned.
- Provides leadership coverage and administrative support during emergencies, staffing challenges, and other operational situations.
- Escalates significant clinical, resident-safety, staffing, or operational concerns to the appropriate senior leadership.
- Monitors clinical practices and resident outcomes to identify opportunities for improvement.
- Assists with reviewing incidents, accidents, falls, medication concerns, changes in resident condition, hospital transfers, and other significant clinical events.
- Works collaboratively with Nursing and Quality leadership to identify trends and implement appropriate corrective and preventive measures.
- Supports ongoing compliance with CMS, NYS Department of Health, and other applicable regulatory and accreditation requirements.
- Promotes continuous survey readiness throughout the facility.
- Conducts rounds, audits, observations, and other monitoring activities to identify potential areas of noncompliance.
- Collaborates with Nursing leadership to monitor staffing levels, assignments, scheduling, attendance, and appropriate utilization of clinical personnel.
- Provides coaching, feedback, and guidance to nursing staff and frontline clinical leaders.
- Serves as a resource for residents and families regarding clinical or operational concerns.
- Performs other duties and assumes additional responsibilities as assigned by the Chief Nursing Officer/Director of Nursing or facility leadership.
Qualifications:
- Current NYS Registered Nurse License
- Minimum of 3–5 years of progressive clinical leadership or nursing management experience, preferably within a skilled nursing facility, long-term care, rehabilitation, hospital, or comparable healthcare environment.
Education:
- Bachelor’s degree in nursing, Healthcare Administration, Healthcare Management, or a related field
- Master's degree in nursing, Healthcare Administration, Business Administration, or a related discipline preferred for positions with broader administrative responsibility preferred
