The Clinical Associate is a Registered Nurse who provides clinical oversight, case management, and field support for BAYADA Assistive Care/Home Care clients in the home setting. Working closely with the Director, Service Supervisor, Client Services Manager, and office team, the Clinical Associate helps ensure safe, compassionate, high-quality care that supports each client’s dignity, independence, and quality of life at home.

This role serves as a clinical resource for caregivers and office staff, conducts client assessments and required supervisory visits, develops and oversees individualized care plans, and coordinates services to support positive outcomes. In partnership with the Service Supervisor, the Clinical Associate also contributes to caregiver development, client satisfaction, workforce stability, regulatory compliance, and the achievement of BAYADA’s mission and Assistive Care operational goals.

Responsibilities

  • Conduct comprehensive in-home client assessments and complete the care planning process, including initial and ongoing reassessments in accordance with BAYADA policy and payer requirements.

  • Develop, implement, and update individualized care plans that reflect each client’s goals, functional status, risks, preferences, and social determinants of health. Ensure written and electronic care plans are accurate and aligned.

  • Provide ongoing clinical and case management oversight for an assigned caseload, ensuring services are delivered safely, effectively, and in accordance with BAYADA standards and applicable state and federal regulations.

  • Collaborate with physicians and other health care providers to coordinate care, obtain and document verbal orders, and ensure timely and accurate completion of required clinical documentation.

  • Conduct reassessments and supervisory visits at required frequencies. Document changes in condition, identified risks, interventions, and follow-up needs, escalating complex or high-risk situations to clinical leadership as appropriate.

  • Support continuity of care by identifying barriers, coordinating appropriate services, and helping clients remain safely at home.

  • Provide clinical supervision, guidance, and professional development for home health aides and other field employees, including skills assessments, competency evaluations, and education in the home and office settings.

  • Partner with the Service Supervisor to promote caregiver success, engagement, retention, and consistent delivery of quality care.

  • Provide individualized coaching and feedback during client visits, reinforcing safety protocols, care procedures, documentation expectations, and BAYADA policies. Document follow-up actions and performance trends.

  • Identify high-potential caregivers and collaborate with the office team to connect them with training, scholarship, recognition, and career development opportunities.

  • Foster a culture of appreciation and engagement by actively using BAYADA Celebrates, BAYADA Bucks, and other recognition tools to support motivation and retention.

  • Build and maintain strong relationships with clients and families through clear, compassionate, and proactive communication.

  • Reinforce BAYADA’s commitment to dignity, independence, and quality of life at home in every client and family interaction.

  • Collaborate with the Service Supervisor and Client Services Manager to support appropriate caregiver-client matches, address client and employee concerns, and strengthen relationships that contribute to service stability and long-term retention.

  • Maintain and use knowledge of community resources and ancillary services—such as adaptive equipment, transportation, and social supports—to address needs beyond the care plan.

  • Help clients and families understand services, expectations, changes in condition, and available resources.

  • Work closely with the Director, Service Supervisor, Client Services Manager, and office team to coordinate caseloads, staffing needs, employee assignments, visit schedules, and service delivery priorities.

  • Participate actively in team meetings, including daily standups, weekly forward/Week-in-Review meetings, case conferences, and other forums used to review high-risk clients, open referrals, and clinical or operational priorities.

  • Use sound clinical and operational judgment to prioritize work, manage the assigned caseload, and support office initiatives and quality improvement activities, including audits, incident follow-up, and risk mitigation planning.

  • Complete clinical documentation accurately and on time in BAYADA systems, including AlayaCare/Viv, to support compliance, billing, payroll, and reporting requirements.

  • Partner with the Service Supervisor on client and field employee needs, including care planning, caregiver support, service issue resolution, client satisfaction initiatives, and efforts that promote continuity of care and a positive service experience.

  • Model The BAYADA Way and the Assistive Care Top 10 Expectations in all interactions with clients, families, caregivers, and teammates.

  • Demonstrate compassion, excellence, and reliability while promoting a culture of safety, inclusion, accountability, and continuous improvement.

  • Encourage open communication, feedback, and collaboration across roles and offices.

Requirements

  • Graduate of an accredited school of nursing.

  • Current Registered Nurse license in good standing in the state of Arizona.

  • Two or more years of recent clinical experience preferred, particularly in home care, medical-surgical nursing, long-term care, or a related setting.

  • New graduates and newly licensed Registered Nurses are encouraged to apply.

  • Experience with client assessments, care planning, clinical documentation, caregiver supervision, or care coordination is helpful but not required.

  • Situational Adaptability: Adjusts schedule, approach, and communication style as client needs, acuity, and office priorities change.

  • Cultivates Innovation and Drives Engagement: Identifies practical solutions that help clients remain safely at home and strengthen caregiver and client engagement.

  • Monday through Friday, 8:30 a.m. to 5:00 p.m.

  • In-office role with regular travel to client homes for assessments, reassessments, and supervisory visits.
