The Opportunity

Boost Home Healthcare is a Medicare-certified, ACHC-accredited agency in Brookline. We are looking for an exceptional RN Case Manager to join our ground-floor team. If you want supplemental income without the red tape of a large corporation, this role is built for you.

What you’ll be doing:

Primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.
Conduct initial evaluations and develop individualized plans of care.
Complete accurate OASIS assessments and timely documentation.
Coordinate care planning with physicians and our clinical team.
Provide education, resources, and emotional support to patients and their families to promote independence and wellness.

What we’re looking for:

Licensure: RN (Registered Nurse) with active MA state license.
Experience: At least 1 year of experience in home health or case management preferred. Expertise in administering IVs and medications.
Reliability: Valid MA driver’s license and reliable transportation.
Availability: Able to commit 10 - 20 hours per week, flexible hours
Knowledge: Familiarity with Medicare OASIS (training is available).
Skills: Strong critical thinking, communication, time management, and the ability to work independently.
Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).
Reliable Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle.
