Responsibilities
~2 min readThe LPN – Patient Program Coordinator is responsible for coordinating day-to-day program operations and overseeing patient flow to ensure an efficient, organized, and positive patient experience. This position serves as a key liaison between patients, physicians, clinical staff, and program management, facilitating communication and coordination throughout the patient care process.
- →Oversee daily patient flow and coordinate the movement of patients through the program to support timely and efficient care.
- →Serve as a primary liaison between patients, physicians, clinical staff, and the program manager.
- →Facilitate communication regarding patient needs, appointments, scheduling, and program-related concerns.
- →Support physicians and clinical staff with the coordination of daily program activities and patient needs.
- →Assist the program manager with day-to-day operations, workflow, scheduling, and administrative functions.
- →Monitor program activities and identify operational issues that may affect patient flow or the patient experience.
- →Coordinate with internal departments and external providers as needed to ensure continuity and efficiency of services.
- →Maintain accurate patient and program documentation in accordance with organizational policies and regulatory requirements.
- →Provide patients and families with information regarding program services, processes, and resources.
- →Assist with resolving patient concerns and escalate clinical or operational issues appropriately.
- →Collaborate with the clinical team to ensure patient needs are communicated and addressed in a timely manner.
- →Support the implementation of program policies, procedures, quality initiatives, and process improvements.
- →Maintain confidentiality and comply with HIPAA, organizational policies, and applicable healthcare regulations.
- Current, active New Jersey Licensed Practical Nurse (LPN) license required.
- Graduate of an accredited practical nursing program.
- Minimum 2 years of clinical nursing experience; outpatient or ambulatory care experience preferred.
- Experience with patient care coordination, patient flow, scheduling, or clinical operations preferred.
- Strong knowledge of patient care processes, clinical documentation, HIPAA, and confidentiality requirements.
- Proficiency with electronic health record (EHR) systems and standard computer applications.
- Excellent organizational, communication, and interpersonal skills.
- Ability to prioritize multiple tasks, resolve routine issues, and appropriately escalate clinical or operational concerns.
- Demonstrated ability to work effectively with patients, families, physicians, nurses, and multidisciplinary healthcare teams.
- Strong commitment to patient-centered care, teamwork, and quality improvement.
- Ability to work effectively in a fast-paced healthcare environment and adapt to changing program needs.
