Home Health Registered Nurse - PRN

We're currently seeking a Registered Nurse to join our ever-expanding Home Health team in the Taylorstown, PA  and surrounding areas This role offers the chance to directly provide patient care within homes, truly making a difference in the lives of our patients.

• →Conducting Start of Care comprehensive assessments and completing OASIS timely
• →Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
• →Preparing accurate, orderly, and timely documentation
• →Coordinating care with patient and provide proper reports to Case Manager
• →Provides health care instructions to the patient/family as appropriate per assessment and plan

• An active RN license in the state in which practicing or New Grad with eligibility to take NCLEX.
