Are you passionate about helping patients navigate their healthcare journey? We are seeking a compassionate, detail-oriented Therapy Referral Coordinator to join our team. In this role, you will play a vital part in coordinating therapy and specialty care referrals, ensuring patients receive the services they need in a timely and efficient manner. 

 

Responsibilities

  • →Assist physician and nurse in preparing therapy referrals in a timely manner.
  • →Work closely with therapy providers to provide necessary referrals.
  • →Verify Medicaid eligibility/PCP assignment for any non-same day request and review complete eCW records to verify appropriateness of referral
  • →Document all communication and maintain accurate documentation in Ecw.
  • →Perform any other related duties as required or assigned.
