RN Case Managers work collaboratively with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination. Essential to this role are the core values of patient advocacy, holistic assessment, care planning, and the empowerment of patients and families to self-manage. This is a great team to be a part of that will provide you with the opportunity to assist with
the coordination of next level of care for our patients.
