Our dedicated team-clinicians, nurses, therapists, and support staff-works collaboratively to provide comprehensive services that promote healing and recovery.
As a Case Manager in the PATH (Projects for Assistance in Transitioning from Homelessness) program, you will play a vital role in helping individuals secure stable housing and access essential community supports. Your work will include:

Building relationships with clients and assisting them in navigating housing options

Connecting individuals to entitlements, benefits, and supportive services

Conducting community outreach to identify people who may qualify for PATH services

Collaborating with service providers, treatment teams, and natural supports

Participating in the development of Individual Recovery Plans (IRPs)

Providing interventions that promote empowerment, independence, and self‑advocacy

Using Motivational Interviewing (MI) to effectively engage and support clients
Your efforts will help remove barriers, strengthen recovery, and create pathways toward long‑term stability.
To be considered for this role, you should have:

A Bachelor's Degree in a mental health‑related field from an accredited institution
Strong knowledge of community resources and local agencies
Experience advocating for supportive services (preferred)
Excellent communication and engagement skills
