Responsibilities

  • →Partner with providers and medical assistants to coordinate referrals that connect underserved patients to essential care such as physical therapy, vision services, and orthopedic specialists.
  • →Carefully assess insurance coverage, patient location, and urgency to remove barriers and ensure timely access to care.
  • →Send referrals securely and accurately, so outside providers have what they need to treat patients.
  • →Support care teams by promptly prioritizing and responding to referral requests, both in person and through instant messaging.
  • →Keep patient charts complete and up to date by scanning, documenting, and organizing intakes and referrals.

Requirements

  • High school diploma or GED required.
  • Strong attention to detail with a consistent focus on accuracy.
  • Professional communication skills when interacting with patients, hospitals, and team members.
  • Ability to work collaboratively to support patients and achieve shared goals.
  • Proficient computer skills: typing, printing, Microsoft Outlook for email and calendar management
  • Demonstrates professionalism through reliability, respectful interactions, clear communication, and effective handling of workplace challenges.
  • Ability to work both independently and part of a team.

Compensation: $21.00 - $26.60/hour. The posted range represents the full salary range for this position. Starting pay is based on factors such as directly related experience, education, licensure and/or certification, and internal equity.
