Oral and Maxillofacial Surgeon

• →D.M.D. or D.D.S. degree from an accredited institution
• →Completion of an accredited residency program in Oral and Maxillofacial Surgery or integrated MD/OMFS pathway
• →Licensed (or eligible for licensure) in the state of Wisconsin
• →Current CPR, ACLS and PALS Certification
• →DEA Registration
• →WI Anesthesia/Sedation Permit for scope of practice
• →Board Certified or eligible to be certified by the American Association of Oral and Maxillofacial Surgery preferred
