Position Summary:


Works as a member of the healthcare team who assists the primary care providers in managing the overall health of their patients.  Provides brief consultation, assessment, intervention and education services to recognize and treat behavioral health issues within primary care.  The goal of the Behavioral Health Consultant is to assist in helping to improve recognition, treatment and management of mild psychosocial/behavioral problems and medical conditions in the patient population. 


Qualifications



 Education: Behavioral Health Nurse Practitioner, Licensed Clinical Psychologist, Licensed Clinical Social Worker,   Licensed Professional Therapist, or Licensed Marriage & Family Therapist.

 Years of Experience: One (1) year minimum practicing in a primary health care setting.

 Licenses and Certifications: Current State of Pennsylvania License,

  Board Certified in Specialty

Knowledge, Skills and Abilities:


  • Excellent written and verbal communication skills
  • Spanish speaking strongly preferred
  • Experience working in primary care and/or community health setting
  • Ability to work harmoniously as part of a health care team
  • Demonstrates effective and efficient computer skills including spreadsheet, word and graphics in order to support data collection
  • Positive, enthusiastic and tolerant demeanor while directly working with patients and health care professionals
  • Eager for experience with Motivational Interviewing, CBT, Acceptance and Commitment Therapy, and solution-focused clinical interventions
  • Possession of a valid drivers’ license and a licensed automobile, preferred
  • Ability to meet approved minimal physical and medical standards
  • Self-motivated and able to work independently
  • Flexible, creative, and eager to work in a fast paced, dynamic environment
  • Ability to define problems, collect data and analyze results.

 

  Scope of Responsibilities:

 

  • Work as a member of the healthcare team providing behavioral health services to pediatric and adult patients with behavioral health issues, substance abuse disorder and/or chronic medical conditions.
  • Perform targeted strengths-based assessment and evaluation, including diagnostic impressions and functional status focused on the presenting problem.
  • Formulate and implement brief behavioral health interventions for a wide range of behavioral and health issues appropriate to the primary care setting.
  • Utilize a variety of treatment modalities including but not limited to Motivational Interviewing, Cognitive Behavioral Therapy, Mindfulness, Solution Focused and Acceptance and Commitment Therapy.
  • Facilitate smoking cessation sessions with patients, including the implementation of evidence-supported psychological treatments (i.e. motivational interviewing and cognitive behavioral therapy), nicotine replacement therapy, and data collection.
  • Provide timely and succinct feedback to providers regarding consultation findings and recommendations.
  • Concise documentation in the patient’s electronic medical record.
  • Triage and refer patients to specialty mental health care, when appropriate.
  • Provide brief follow-up, including relapse-prevention education.
  • Develop, teach and/or provide oversight for classes that promote education and skill-building to enhance psychological and physical health.
  • Provide on-going consultation services for a sub-set of patients who require on-going monitoring and follow-up.
  • Share knowledge with other team members and patients, both formally (in-services, consult responses) and informally (hallway conversations).
  • Develop collaborative relationships with external medical, behavioral and social service agencies.
  • Maintains communication with insurance companies for patient eligibility and claim denials.
  • Understanding of policies and procedures of organization to relate to clients, when necessary.
  • Travel between offices as required.
  • Completes other duties as assigned by supervisor.

 

 Physical demands:

  • Moderate physical effort in carrying papers, forms, supplies and equipment.

 

Work environment:

  • The primary work environment is a busy multi-cultural ambulatory care setting. There is routine exposure to infectious diseases and blood borne pathogens.
