Make a Lasting Impact - Join Us as a Home Health Care Manager
Are you passionate about helping children and families access the services and supports they need to thrive? We are seeking a dedicated Care Manager to coordinate and oversee comprehensive care management services for an assigned caseload. In this rewarding role, you will serve as a trusted advocate, helping children and their caregivers navigate the healthcare and social service systems while promoting wellness, independence, and positive outcomes.
The Care Manager works collaboratively with families, healthcare providers, schools, community organizations, and other service partners to ensure clients receive coordinated, person-centered care tailored to their unique needs.
Responsibilities
~1 min read- →Obtain required Care Management enrollment consents from clients and/or legal guardians.
- →Complete initial and ongoing Child and Adolescent Needs and Strengths (CANS) assessments to determine appropriate levels of care management.
- →Develop, implement, and monitor individualized Plans of Care.
- →Coordinate services based on client needs and acuity levels.
- →Support adherence to treatment recommendations and service plans.
- →Monitor and evaluate medical, behavioral health, wellness, social, and community support needs.
- →Conduct required client contacts and face-to-face visits in accordance with program standards and levels of care.
- →Maintain timely, accurate, and compliant documentation utilizing MAPP and Electronic Health Records (EHR) systems.
- →Advocate for clients and families within the agency and across community service networks.
- →Connect clients to healthcare, behavioral health, educational, housing, and community resources.
- →Educate children and caregivers regarding chronic conditions, preventive care, immunizations, screenings, and wellness practices.
- →Assist families in obtaining and maintaining essential public benefits, including:Medicaid, SNAP, Cash Assistance, Social Security benefits, Housing supports, Legal services, Employment and training resources.
- Bachelor Degree of Arts or Science with two years of experience or
- Registered Nurse or Licensed Practical Nurse with one year of experience.
- Bilingual is a plus.
Requirements

- Experience providing service coordination and information, linkages, and referrals for community-based services.
