Discharge Planner

Under general supervision coordinates the integration of Discharge Planning & Swing Bed level care. This position functions into the patient care, discharge and home planning processes with other hospital departments, external service organizations, agencies, and health care facilities; provides direct care to selected patients as appropriate; ensures compliance with quality patient care and regulatory standards.

Social worker or RN preferred, willing to train
At least one (1) year experience working with medical patients preferred
Must possess assessment skills, exhibit sound judgment and be able to communicate effectively
Must be knowledgeable of community resources and be able to network effectively within the community
Must be knowledgeable regarding Medicare and AHCCCS regulations/requirements
