We are seeking an experienced Director of Patient Care Services (DPCS) to lead our hospice clinical team and ensure exceptional, compassionate patient care. This is an outstanding opportunity for a proven hospice nurse leader who is passionate about clinical excellence, staff development, regulatory compliance, and delivering high-quality end-of-life care.
As the clinical leader of the organization, you will oversee all patient care services, provide leadership to interdisciplinary clinical teams, ensure compliance with state and federal hospice regulations, and help drive quality outcomes for our patients and families.
Responsibilities
~1 min readAs the Director of Patient Care Services, you will:
- →Provide leadership, supervision, and support for all hospice clinical staff, including RNs, LVNs/LPNs, and interdisciplinary team members.
- →Ensure high-quality, patient-centered care is delivered in accordance with each patient's individualized plan of care.
- →Maintain compliance with Medicare Hospice Conditions of Participation (CoPs), state hospice regulations, and agency policies.
- →Oversee clinical staffing, scheduling, and nurse caseloads to ensure safe patient care and regulatory compliance.
- →Develop and manage the agency's patient acuity system to ensure appropriate staffing and balanced clinical workloads.
- →Lead and facilitate the Interdisciplinary Group (IDG) process while collaborating closely with the Medical Director regarding patient care, symptom management, and clinical decision-making.
- →Monitor clinical documentation, chart accuracy, and medical record compliance.
- →Lead clinical onboarding, orientation, competency assessments, continuing education, and staff development initiatives.
- →Participate in Quality Assurance and Performance Improvement (QAPI) activities, clinical audits, and adverse event reviews.
- →Ensure survey readiness and serve as a clinical leader during state and federal regulatory surveys.
- →Designate and mentor a qualified DPCS designee to provide leadership coverage as needed.
- →Foster a culture of compassion, accountability, teamwork, and continuous improvement.
Requirements
~1 min read- Current, unrestricted Registered Nurse (RN) license.
- Bachelor's degree in Nursing or a related healthcare field and at least 2 years of supervisory or management experience in hospice or home health within the past five years;
OR
- Minimum of 4 years of supervisory or management experience in hospice or home health within the past five years.
- Strong working knowledge of hospice regulations, Medicare Conditions of Participation, and state licensing requirements.
- Experience leading interdisciplinary clinical teams.
- Excellent leadership, communication, coaching, and organizational skills.
- Ability to effectively manage multiple priorities in a fast-paced clinical environment.
- No disciplinary action against a professional license within the past seven years.
- Successfully pass OIG exclusion screening and required health screenings/immunizations.
- Previous experience serving as a Director of Patient Care Services or Hospice Clinical Director.
- CHPN certification or other hospice certification.
- Experience with QAPI initiatives, survey preparation, and regulatory compliance.
- Strong knowledge of hospice EMR systems and clinical operations.
