Right at Home is looking for a Client Care Liaison to join our San Luis Obispo team. This is a hands-on, relationship-focused role for someone who can connect with clients and families, support caregivers, manage changing care needs, and help make sure we are consistently providing the level of care we promise.

We are a locally owned in-home care agency with a people-first culture and high expectations for the care and service we provide. Our office moves quickly, priorities can change throughout the day, and no two clients are exactly alike. We’re looking for someone who can stay organized, communicate well, use good judgment, and remain calm and solution-focused when things don’t go exactly as planned.

What You’ll Do

As our Client Care Liaison, you will help oversee the client experience from the start of care and throughout the client relationship. You’ll work closely with clients, families, caregivers, office staff, referral partners, and other professionals involved in a client’s care.

Responsibilities include:

  • Conduct client assessments and help develop, implement, and update individualized care plans.

  • Maintain regular communication with clients and families to ensure services continue to meet their needs and expectations.

  • Monitor the quality and appropriateness of care being provided and address concerns when they arise.

  • Identify changes in a client’s condition, needs, or circumstances and communicate those changes to the appropriate individuals.

  • Help ensure caregivers are appropriately matched and assigned based on each client’s individual needs.

  • Support and mentor caregivers in delivering safe, compassionate, high-quality care.

  • Investigate client concerns, service issues, incidents, policy violations, and other reportable events, and assist with appropriate follow-up and resolution.

  • Maintain accurate and timely client records and documentation.

  • Help clients and families identify community resources that may support continuity of care.

  • Build positive relationships with referral partners and other professionals throughout the community.

  • Represent Right at Home professionally at community events, meetings, and programs as needed.

  • Work collaboratively with our office team to solve problems and create the best possible experience for our clients and caregivers.

Who We’re Looking For

This position requires someone who is comfortable taking ownership. You should be able to work independently while also communicating closely with the rest of the team.

Our ideal candidate:

  • Is compassionate, personable, and comfortable working directly with seniors, adults with care needs, and their families.

  • Has strong communication and relationship-building skills.

  • Is highly organized and able to manage multiple clients, priorities, and deadlines.

  • Uses sound judgment and is comfortable making decisions and solving problems.

  • Can handle difficult conversations professionally and respectfully.

  • Remains calm and positive in a fast-paced environment.

  • Is comfortable working both in the office and out in the community.

  • Has basic computer and office skills and is comfortable learning new systems.

  • Has previous experience in home care, healthcare, senior care, case management, or a medical office environment; experience in these areas is strongly preferred.

Requirements

  • High school diploma or equivalent required.

  • Minimum of two years of professional work experience.

  • Valid California driver’s license.

  • Reliable, insured personal vehicle and ability to travel throughout San Luis Obispo County for client visits and other business needs.

  • Ability to meet applicable health requirements for employees who have direct contact with clients.

  • Ability to read, write, speak, and understand English as required to perform the responsibilities of the position.

  • Availability for occasional on-call responsibilities outside of normal office hours.

Compensation & Benefits

  • $22–$28 per hour, based on experience and qualifications.
