This is a full time, benefits eligible, grant funded opportunity. Current funding will expire on 7/31/2027, with the possibility of extension

The Program Consultant will support Year 3 of a national American Heart Association initiative focused on building the capacity and competencies of health care providers. This role will help develop, implement, and evaluate community-focused interventions in primary care settings that use evidence-based strategies to increase education and awareness of heart valve disease, advance quality improvement efforts, and test innovative approaches for identifying, educating, supporting, and referring patients to appropriate specialty and definitive care

The program consultant will lead and support a multi-market learning collaborative comprised of heart valve centers across three geographic regions, while engaging primary care clinicians, health systems, professional societies, patient advocacy organizations, and other key stakeholders to advance heart valve disease (HVD) awareness, identification, diagnosis, referral, and treatment. Responsibilities include facilitating collaborative discussions, gathering and integrates stakeholder insights, identifying barriers and opportunities within care pathways, and translating findings into innovative educational resources, tools, and dissemination strategies for health care professionals and patients. The Program Consultant will work closely with internal teams, scientific advisors, clinical experts, and external partners to develop and implement evidence-based interventions, support quality improvement efforts, evaluate program outcomes, and contribute to scalable solutions that strengthen community systems of care for HVD. Strong relationship management, strategic thinking, and project management skills are essential, as the position is responsible for delivering project activities within scope, budget, and timeline while advancing national learning and dissemination efforts.

• →Leads the planning, execution, and continuous improvement of a Heart Valve Center Learning Collaborative across three pilot markets. Facilitates virtual and in-person meetings, develops agendas and learning plans, and promotes peer-to-peer model sharing of successes, challenges, and innovative approaches to care delivery.
• →Collaborates with internal teams, clinical subject matter experts, scientific advisors, and external partners to design, implement, and disseminate evidence-based educational resources, tools, and model shares. Analyzes stakeholder feedback and market insights to inform educational strategies, resource development, and program optimization.
• →Manages cross-functional project activities and stakeholder engagement efforts to ensure successful delivery of initiative objectives. Oversees timelines, deliverables, reporting, and outcome measurement while identifying opportunities to scale effective practices and strengthen referral pathways between primary care and specialty care providers.

• Bachelor’s Degree or equivalent work experience
• 3 years of relevant experience
• Experience in quality improvement methodology and frameworks
• Ability to work independently with minimal supervision as well as be a collaborative team player
• Ability to be dynamic, detail-oriented, multi-task, and re-prioritize in a fast-paced environment
• Strong problem-solving skills
• Skill and ability in providing customer service at an excellent level to internal and external customers and partners
• Strong interpersonal/relationship-building skills
• Excellent oral and written communication
• Ability to travel up to 25% local and overnight stay

• Experience in health-related fields
• Quality improvement experience
• Health education experience
• Health Center quality improvement, operations, or clinical experience
• Experience building and cultivating volunteer and client relationships
• Experience facilitating virtual and in-person meetings
