~1 min readAs a Case Manager, you'll work alongside young people aged 15 years and over who are preparing to leave care, as well as young adults who have already exited care.
You'll provide outreach-based case management that is practical, collaborative and tailored to each young person's strengths, aspirations and individual needs. Your role will focus on helping young people achieve sustainable outcomes across:
- Housing and accommodation
- Education and employment
- Health and wellbeing
- Independent living skills
- Community participation
- Family, cultural and social connections
You'll coordinate services, advocate for young people, develop goal-focused plans, and work closely with Child Safety, community organisations and other stakeholders to ensure young people have the support they need to thrive into adulthood.
This role requires regular travel throughout the service area and offers the opportunity to work independently while remaining well connected to a supportive and experienced team.
We're looking for someone who is passionate about supporting young people and confident building strong, trusting relationships.
You will ideally have:
- A relevant Degree in Social Work, Psychology, Human Services, Social Science or a related discipline (or equivalent experience)
- Experience providing case management to young people with complex needs
- Strong engagement, assessment, planning and advocacy skills
- Experience coordinating services and working collaboratively with internal and external stakeholders
- Excellent communication and documentation skills
- The ability to work independently, manage competing priorities and maintain accurate case records
- An understanding of trauma-informed and strengths-based practice
- Experience supporting young people with a care experience will be highly regarded
- A current unrestricted Queensland Driver Licence
- National Criminal History Check
- Working with Children Check (Blue Card)
- Licenced Care Service Check (LCS)
