Social Worker - Rapid Discharge to Assess

Liquid Personnel is recruiting an Adults Social Worker to join its client’s Rapid Discharge to Assess Team based in Trafford.

• →Timely Hospital Discharge Planning: Work closely with health professionals to assess patients' needs and coordinate safe, efficient discharge plans that avoid unnecessary hospital stays.
• →Holistic Needs Assessment: Conduct strength-based assessments in community settings post-discharge to determine ongoing care and support requirements, ensuring person-centred outcomes.
• →Multi-Agency Collaboration: Liaise with families, care providers, and partner agencies to implement tailored support packages and facilitate smooth transitions from hospital to home or interim care.

To be successful in this role you must have:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
