Join a team dedicated to supporting the community. As a Social Worker Coordinator, you will play an important role in assisting social workers and mental health professionals as they provide critical rehabilitation, psychology, and social work services.

Responsibilities

→Helps social workers in their day to day tasks
→Discuss benefits like LTC, meals, STS, housing to determine need versus wants.
→Review and submit reports regarding any problems with a patient to supervisor.
→Comply with the patient rights and Leon Medical Centers Policies and Procedures.
→Good assessment, listening, and counseling skills.
→Perform Essential Duties as outlined above.
→Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the patients.
→Involve the patient/family in planning social service programs when possible.
→Good communication skills with excellent self-discipline and patience.
→Participate in the community planning related to the interest of the facility and the services and needs of the patient and family.
→Maintains confidentiality of all proprietary and/or confidential information.
