The ideal candidate is outgoing, professional, and comfortable developing referral relationships.

Responsibilities

→Develop and maintain relationships with hospitals, physicians, social workers, discharge planners, and community organizations.
→Promote our nursing home, rehabilitation, and care services to potential referral sources.
→Visit hospitals, medical offices, and community organizations to establish and strengthen partnerships.
→Follow up with referral sources and prospective residents/families.
→Attend community events, health fairs, and networking opportunities.
→Maintain accurate records of outreach activities and referrals.
→Collaborate with the admissions and clinical teams to support census growth.
→Represent the facility professionally and positively in the community.
