Location: Crest View Nursing & Rehabilitation Center - New Lisbon, WI

Schedules Available:

• Full-time, 75 hours per pay period; PM Shift
• Part-time 1, 60 hours per pay period; PM Shift
• Part-time 2, 15 hours per pay period; PM Shift

Weekend rotation: Every other weekend

Holiday rotation: Every other holiday

Additional Information: **Up to $3,000 sign on bonus**

The Nursing Home Licensed Practical Nurse position assesses and passes medication to residents, reports to physician any changes they have, reports to charge nurse, updates family members, and assists CNAs.

• →Administer medications to residents while following the 5 Rights
• →Report any allegations of abuse, neglect and misappropriation to Director of Nursing and Administrator
• →Report any change in condition to the charge nurse
• →Assist CNAs with answering call lights and other responsibilities
• →Update physicians, family members, etc. of change in condition as needed
• →Perform other duties as requested.

• Must hold and maintain Wisconsin LPN license.
• Current CPR/BLS certification required.
• 1+ years of related work experience preferred.
• Exceptional accuracy and attention to detail required.

• Intermediate proficiency with computers is required.
• Knowledgeable regarding medication administration.
• Comprehensive nursing skills.
• Strong quantitative and analytical competency.
• Self-starter with excellent interpersonal communication and problem-solving skills.
• Ability to have continuous oral and written communication with co-workers, patients, and supervisors is essential.
