The Home Health Aide (HHA) / Licensed Nursing Assistant (LNA) is a paraprofessional member of the home care team who provides personal care services to patients under the supervision of a Registered Nurse or therapist.

In this role, you will support patients with daily activities, monitor their condition, and communicate observations to the clinical team to ensure safe, high-quality care.

• →Provide personal care assistance such as bathing, grooming, dressing, and mobility support
• →Observe and report changes in patients’ condition to the supervising nurse or therapist
• →Document care provided and patient observations accurately
• →Follow the care plan established by the Registered Nurse
• →Maintain a safe and supportive environment for patients in their homes
• →Promote quality, continuity, and safety of patient care

• Licensed Nursing Assistant (CNA) in New Hampshire preferred; Home Health Aide certification accepted
• Must be 18 years of age or older
• Ability to read and follow written care plans and document services provided
• Ability to work independently with minimal supervision
• Compassion and empathy for individuals who are ill, injured, or elderly
• Current CPR certification required
• Strong interpersonal skills, patience, and professionalism
• Valid driver’s license and reliable insured vehicle
• Ability to assist patients with repositioning, transferring, and other physical tasks.
