People Incorporated is a human service Agency serving the Southcoast community, including the Greater Fall River, New Bedford, and Taunton areas. We are looking for a energetic and dynamic RN Clinical Trainer/Case Manager to support our 24-Hour Residential Division, regulated by the Massachusetts Department of Developmental Services (DDS), by ensuring that individuals receive high-quality nursing oversight and healthcare coordination to promote optimal health and safety.

This position combines clinical training and nursing development (approximately 75%) with case management responsibilities (approximately 25%) for individuals residing in residential homes. The primary focus of this role is to strengthen nursing practice, ensure Medication Administration Program (MAP) compliance, support regulatory adherence, and provide clinical guidance to residential staff across multiple program locations.

ESSENTIAL FUNCTIONS

Clinical Training and Nursing Development- (Approximately 75%)

  • Serve as the primary trainer, preceptor and clinical resource for new RN and LPN nursing staff
  • Participate in new hire orientation, providing structured training on residential policies, procedures, MAP requirements, and use of agency systems including Icentrix and other required platforms to newly hired nurses and Direct Care Professionals
  • Provide ongoing clinical education, coaching, and competency validation for nursing staff
  • Assist with implementing and maintaining a comprehensive nurse training manual and structured precepting program
  • Develop and utilize competency checklists to validate nursing clinical skills learned during precepting
  • Support Residential staff in strengthening documentation practices, care coordination and regulatory compliance
  • Assist with Medication Administration Program (MAP) training and compliance, including but not limited to G-tube and J-tube competencies, Epi-pen administration, medication routes training and other individual specific trainings related to MAP
  • Support Residential staff in understanding DDS regulations, healthcare protocols, MAP requirements and Agency policies
  • Identify gaps in training and recommend improvements to curriculum, procedures, and competency standards
  • Assist in developing healthcare policies, protocols and ethical guidelines as needed
  • Provide on-site clinical support to Residential programs as needed to address training, compliance, or quality concerns

Case Management- (Approximately 25%)

  • Monitor the general health and well being of program individuals to ensure health needs are met
  • Complete required nursing assessments and documentation, including quarterly reviews and health status updates
  • Conduct routine healthcare audits of assigned individuals to ensure compliance with DDS, MAP and Agency standards
  • Act as liaison between program individuals, healthcare providers, families, DDS and other stakeholders as needed
  • Attending healthcare appointments as needed to support care coordination
  • Assist Program Managers in preparing components of ISPs, Supportive and Protectives plans, health care protocols, and medication treatment plans
  • Collaborate with program management to ensure healthcare orders, medication administration records, protocols, HCSIS entries and any other healthcare related documentation is in compliance with laws, policies and regulations
  • Conduct fall prevention assessments
  • Participate in the admission process for new Residential referrals
  • Assist program managers with hospital and rehab discharge planning to ensure safe transitions
  • Monitor and document code status, healthcare proxy, guardianship, and end-of-life planning information, such as palliative and hospice care
  • Support program managers on annual health and wellness screenings

GENERAL NURSING DUTIES:

  • Ongoing triage during regular work hours
  • Conduct periodic infectious disease testing as needed
  • Travel to residential sites as needed to support training, audits, and case management
  • Use personal vehicle for work-related travel as required
  • Attend nursing department and staff meetings as required
  • Ensure and safeguard individuals’ human dignity, rights and liberties
  • Utilize state and Agency software for tracking, training and MAP
  • Maintain all program and Agency required trainings
  • Attend meetings, trainings, and professional development as required
  • Perform other duties as assigned by management

QUALIFICATION REQUIREMENTS

  • Current unrestricted licensure in Massachusetts as a Registered Nurse (RN) required
  • 2 years’ experience in nursing preferred
  • 1 year experience working with intellectual/developmental disabilities population preferred
  • Current CPR/First Aid (Healthcare provider training)
  • Ability to clearly and confidently communicate with outside medical professionals
  • Demonstrated strong clinical judgment, advanced critical thinking skills, and the ability to practice confidently within the RN scope of practice.
  • Ability to maintain reliable transportation and travel to local areas, including, but not limited to Fall River, Somerset, Swansea, Mansfield, Raynham, Taunton, Assonet, Westport
  • Ability to maintain confidentiality and HIPAA standards
  • Ability to work as a team or independently
  • Ability to clearly communicate with others
  • A qualifying criminal background check, driving record, and valid drivers license required
  • Ability to provide current copy of all required healthcare records and TB assessments
  • Experience or strong interest in precepting and developing nursing staff strongly preferred.
