We are currently seeking a highly experienced and motivated Lead Physiotherapist to join our client’s Care Transfer Hub (CTH) team in Shropshire. This is a pivotal role within a multi-disciplinary discharge planning service, supporting complex patient transitions from hospital to community care.
Responsibilities
~1 min read- →Lead the development and delivery of the Discharge to Assess (D2A) model across acute and community settings.
- →Provide advanced clinical leadership and autonomous decision-making for complex discharge planning.
- →Collaborate with health and social care professionals to ensure seamless, patient-centred transitions.
- →Deliver specialist physiotherapy assessments and interventions across inpatient and community environments.
- →Support governance, quality assurance, and service innovation within the Integrated Discharge Team (IDT).
- →Contribute to strategic planning, workforce development, and training across the CTH.
- →Participate in 7-day service delivery and rotational work with inpatient and community therapy teams.
Requirements
~1 min read- HCPC registration (Health and Care Professions Council)
- Degree in Physiotherapy
- Extensive post-registration experience in acute, community, or social care settings
- Proven leadership in complex case management and discharge planning
- Strong understanding of D2A pathways, NICE guidelines, and intermediate care models
- Experience in MDT working, staff supervision, and service development
- Excellent communication, clinical reasoning, and decision-making skills
- Previous NHS experience essential
