We are currently seeking a highly experienced and motivated Lead Physiotherapist to join our client’s Care Transfer Hub (CTH) team in Shropshire. This is a pivotal role within a multi-disciplinary discharge planning service, supporting complex patient transitions from hospital to community care.

Responsibilities

→Lead the development and delivery of the Discharge to Assess (D2A) model across acute and community settings.
→Provide advanced clinical leadership and autonomous decision-making for complex discharge planning.
→Collaborate with health and social care professionals to ensure seamless, patient-centred transitions.
→Deliver specialist physiotherapy assessments and interventions across inpatient and community environments.
→Support governance, quality assurance, and service innovation within the Integrated Discharge Team (IDT).
→Contribute to strategic planning, workforce development, and training across the CTH.
→Participate in 7-day service delivery and rotational work with inpatient and community therapy teams.

Requirements

HCPC registration (Health and Care Professions Council)
Degree in Physiotherapy
Extensive post-registration experience in acute, community, or social care settings
Proven leadership in complex case management and discharge planning
Strong understanding of D2A pathways, NICE guidelines, and intermediate care models
Experience in MDT working, staff supervision, and service development
Excellent communication, clinical reasoning, and decision-making skills
Previous NHS experience essential
