Job Description:
This position is responsible for establishing and maintaining performance standards and other metrics for
evaluation of LES case management with focus on IDEA, Part C State Performance Plan/Annual Report
Indicators. This includes interpreting and putting in place standards for measuring timelines of services
,Individual Family Support Plans (IFSP)outcomes, and compliance with the statutorily required
performance standards. Develop and ensure processes are implemented to evaluate and identify
opportunities for improvement in the provision of high-quality services offered in accordance with IDEA.
Work closely with Case Manager Supervisor to address any non-compliance relative to case management.
Execute process improvements solutions to compliance barriers, using data analysis and optimization tools
to support strategies for improvement plans. Assist with pre-service and ongoing training of case
managers.
 
This position performs two primary case management functions: intake and referral
responsibilities and temporary Case Manager coverage during staff absences,
vacancies, or periods of high caseloads. Intake and referral responsibilities include
conducting initial contacts with families, explaining the Early Steps Program’s philosophy,
eligibility and evaluation processes, documenting family concerns, updating child and
family information, providing education regarding developmental issues, completing
developmental screenings when necessary, and coordinating evaluations and other next
steps. When providing temporary case management coverage, the position delivers
targeted case management services, including facilitating the development, review, and
revision of Individualized Family Support Plans (IFSPs) with families and multidisciplinary
team members. The position ensures services are family-centered, culturally responsive,
and consistent with the Early Steps natural-environments principle, while educating
families about procedural safeguards, service-delivery options, transition planning, and
available community resources. The position collaborates with providers and community
partners to coordinate evaluations, authorizations, and service implementation and
maintains accurate and timely documentation. Assigned cases are monitored for
compliance with all federal and state timelines, including the 45-day timeline and
six-month and annual review requirements. The position also completes required data
entry and billing for targeted case management activities. Proficiency with statewide data
systems, Medicaid billing procedures, and Early Steps documentation standards is
essential.
 
This position is required to participate in in-service training programs as specified by the State Early Steps
Program. Completion of this requirement must be documented in the coordinator's personnel file.
 
This position acts as a community liaison for the Early Steps Program, networking with other providers of
early intervention services, including the public-school systems, private providers, and the medical system.
This requires attendance at interagency.
Expected Salary:

$58,000-$63,000

   
Preferred Qualifications

•  Must meet State requirements for Service Coordinator
•  Knowledge of  IDEA, Part C Early Intervention Policies
•  Working knowledge of child development, family dynamics, developmental disabilities, and factors which place infants/ toddlers at risk for developmental delays
•  Skill in working with young children and their families in collaborating with parents and other professionals.
•  Experience writing outcome and qualitative note taking abilities
•  Experience working with community support systems and resources
•  Experience with quality assurance
•  Communication and interpersonal skills which are effective, respectful, and sensitive with respect to a varied and multicultural population of children, families, and co-workers
•  Skills in problem-solving, mediation, conflict resolution and teamwork are essential for collaboration as a team member, as well as for working with families and community agencies
•  Strong computer skills. Comfortable in using Microsoft Suite: Outlook, Excel, Word and PowerPoint and databases
•  Ability to function under pressure in a fast-paced human service environment
•  Must be capable of dealing with periodic cyclical workload pressures and levels of responsibility
•  Planning/Organizing - Prioritizes and plans work activities; Uses time efficiently; Develops realistic action plans.
